2025 Player Information Registration

All Rostered Soccer Players (Boys, Girls, JV and Varsity) are required to go the following google forms link and register their player information on this 2025 Lakeside Vikings Soccer Tryout Form.

Mandatory Player Information

All LHS Soccer Players trying out must do the following items below…

LHS has partnered with Dragonfly Max to automate the management of required athletic health forms.  Go to https://max.dragonflyathletics.com/ to setup your account.  The code for Lakeside High School is YPDBBB.  In the dragonfly site, you will be able to upload your completed physical and electronically sign the additional required forms.

Required forms:  need to be submitted on dragonfly before tryouts or ANY practice with the team

  • Pre-participation sports physical - (click here for blank copy to take to physician - en español ). NOTE: Provide a current physical performed within the last year (performed on January 3rd 2024 or later). Only the last page with doctor signature/stamp and date is necessary to upload (not all 4 pages).

    Our sponsor, Atlanta Urgent Care (2738 Clairmont Rd, Atlanta, GA 30329) provides $40 walk-in Physicals (no appointments necessary).

This is a reminder that student accident insurance is offered through T.W. Lord & Associates.  This coverage provides protection against medical expenses resulting from accidental injury to your child.  The link contained on the District's website to the specifics regarding this program is:  https://www.dekalbschoolsga.org/documents/accident-insurance/student-accident-insurance-(english).pdf (or Spanish here)   A similar link can be found on your child's school website. For elementary and middle school sites the link may be accessed by selecting most of the main vertical links on the left, then locating the Student Accident Insurance tab at the bottom. Parents may click on the About Us tab on high school websites to access the link.
